Consumer Health Data Privacy Policy

We never sell your consumer health data, we never give your quiz answers to another company, and we do not use your health information to advertise to you.
1. What this policy covers
This policy applies to consumer health data that Rebuild Your Vision, LLC ("Rebuild Your Vision," "we," "us" or "our") collects through rebuildyourvision.com, our store, the Vision Quiz, our email and our customer support. It adds to our Privacy Policy. Where the two differ, the one that protects you more applies.
"Consumer health data" means personal information that is linked or reasonably linkable to you and that identifies your past, present or future physical health, or could be used to infer it. For us, that includes things like the eye conditions you tell us about, your Vision Quiz answers, and the fact that you bought an eye vitamin or read about a particular eye condition, when that information is connected to you. Information that cannot reasonably be linked to you is not consumer health data.
2. The consumer health data we collect, and why

We collect consumer health data only for these purposes. You can take the Vision Quiz and see your result without giving us your email address, and nothing you answer is connected to you unless you choose to give it.
3. Where it comes from
- From you, when you take the Vision Quiz, fill in a form, place an order, click a topic link in one of our emails, or contact us.
- From your browser, when you visit our website (Privacy Policy, Section 4).
- From our payment and shipping providers, about your orders.
4. We never sell it, and we do not use it for advertising
We do not sell consumer health data or exchange it for anything of value. We do not give it to other companies or affiliates for their own marketing, and we do not use it to advertise to you. We have no affiliates that receive it.
Your Vision Quiz answers are stored on our own systems. They are not sent to Google Analytics or to any advertising company. Our analytics receive only that a quiz was started, which step was viewed and whether it was finished. Google Analytics does receive the addresses of the pages you visit on our website, which can include articles about eye conditions (Privacy Policy, Section 4).
5. Service providers that process it for us
These companies process consumer health data to provide their service to us. Google may also use the website activity it receives under its own policies (Privacy Policy, Section 4).

We may also disclose consumer health data when the law requires it.
6. Your rights
Whether or not a law in your state requires it, you may:
- ask us to confirm whether we collect, share or sell your consumer health data, and see it;
- ask for a list of the companies we have shared it with, with their contact information;
- withdraw your consent at any time;
- ask us to delete it; and
- appeal if we turn down your request.
We will not treat you differently for using these rights.
7. How to make a request
Contact Customer Support, call (877) 733-4543, email support@rebuildyourvision.com, or write to us at the address below. Tell us what you would like us to do and the email address you use with us. You do not need to use any special words, and you do not need an account.
To protect you, we may confirm the request from the email address we have on file. Please do not send medical records or identification documents unless we ask. An authorized agent may make a request for you where the law allows.
We respond within 45 days of receiving your request. If we need more time, we may extend that once by up to 45 more days, and we will tell you why within the first 45 days. Requests are free up to twice a year.
8. Deletion
When we delete your consumer health data, we delete it from our active systems and tell our service providers to delete it too. Copies in our backups are deleted within six months, and we make sure deleted data is not restored from a backup into our active systems.
9. Appeals
If we turn down your request, we will explain why and how to appeal. To appeal, reply to our decision or contact us as described in Section 7. We will answer your appeal in writing within 45 days of receiving it. If we deny your appeal, you may contact the attorney general of your state. Washington residents may file a complaint with the Washington State Attorney General at www.atg.wa.gov/file-complaint.
10. How we keep it and protect it
We keep consumer health data only as long as needed for the purposes in Section 2: quiz answers that are not connected to an email address cannot be linked to you and are kept to understand and improve the quiz, and quiz information connected to your email is kept while you remain subscribed or until you ask us to delete it. Access is limited to the people who need it to serve you. We use encrypted connections and restricted, multifactor-protected administrative access.
11. Nevada and Connecticut residents
Nevada residents have the rights described in this policy and may use the same request and appeal process. Connecticut residents also have the rights described in Section 9 of our Privacy Policy.
12. Changes
We will post any change to this policy with a new date. We will not collect consumer health data for a new purpose, or share it in a new way, without first updating this policy and getting your consent.
